- Physicals are required for each Student-Athlete.
- Physicals are valid for 365 days.
- Physicals must be performed by the student's medical home (student's physician, nurse practitioner, physician assistant or nurse specialist).
- These examinations shall not be at the expense of the district.
- These examinations must be on forms furnished by the district. Please use the link above or pick up the forms in the offices of the high school, middle school or director of student activities.
- The examination must be filled in completely, signed, stamped, and dated by your "medical home."
- Please return the examination to the high school or middle school office, school nurse or athletic trainer.
- Once the school receives the examination it will be reviewed and signed by the school's physician.
If your son or daughter had a physical and played a fall sport and now wants to play a winter or spring sport
he/she will need the following:

Obtain a health questionaire and parent permission card from link above or the offices of the high school, middle school, or director of activities. |

Fill out both forms completely and have the appropriate signatures. |
| 3. |
Return the forms to the high school or middle school office, school nurse or athletic trainer. |
Your son/daughter will not need another physical unless it will be order than 365 days during that season.
